1.Those who claim reservation must enclose relevant Caste/Caste Income Certificate.
2.All Catholic & Christians should produce a supporting letter from their respective parish priest/pastor, mentioning their involvement in the church.
General Instruction
- Fill in the complete application in block capital letters only.
- Mention the names as per the transfer certificate or as mentioned in the marks card only.
- Fill in the Date of Birth column in figures ( numeric ) and not in words.
- Do not mention the percentage(%) while writing your percentage of marks.
- You can apply only for one course per application form.
- Do not overwrite ,scratch or use white ink.
- Incomplete application forms or inaccurate answers will be rejected.
- Paste only stamp size photographs & not passport size.Do not staple/pin the same.
- Do not include orginal certificate with this application.(Only attested xerox copies to be enclosed)
